Hi, I switched to WordPress’s default theme and tested it. There is the same issue in the theme. That means; The problem is not related to the theme. First of all, I would recommend you to go to the mailchimp plugin’s support page, open a relevant question there and wait for a response. After the response, we will decide how we should act. Hi, I see you get an error. But in order to be able to detect it, you have to look back at these steps. Because we tested it on our own site and saw that it worked fine.

1. Make sure you have lists created in your MailChimp account.
a. Login into your MailChimp account.
b. Click on the Lists tab from the top menu.
c. Hit the Create List button in the top right corner on the next screen.
d.Fill in your list details: list name, company name, email and MailChimp message. Click Save to save the changes.

2.Now you will need to change your WordPress settings. Please, login into your site backend:
a. Open the MailChimp for Form > MailChimp page.
b. Locate Your MailChimp Account section and click Renew MailChimp lists button.
c. You will see all of available lists there.
d. Open MailChimp for Form > Form page.
e. Select Settings tab. You should see all available lists there.
f. Select the list(s) to which people, who submit this form should be subscribed.

3. Select one and save. This will ensure your user data is added to the list and you would not see any errors.

Thus, the elements will look the same in the list because they will have the same number of the description. We can solve this with CSS code. But it will look more natural and healthy in this way.
